We learn what is happening now, what has already been tried, and whether there are urgent safety concerns.

When appropriate, a qualified clinician completes or reviews the assessment needed for the requested service.

We explain availability, authorization needs, and any recommended intake or referral action in plain language.

Helpful information to include

  • Person's name, age, location, and contact information
  • Reason for referral and the service being requested
  • Current symptoms or behaviors, including frequency and severity
  • Safety concerns and recent crisis or hospital involvement
  • Current diagnoses, providers, medications, and prior services
  • Insurance or Medicaid plan information, when applicable
  • Guardian information and willingness to participate for youth services

For Intensive In-Home referrals

Include clear information about the youth's level of functioning, family and community conflict, prior interventions, risk of out-of-home placement, and why home-based treatment is likely to be more effective than clinic-only care.
